About Your Visit: What to Expect

To help make your visit as productive as possible, please bring the following items to your appointment:

Government-issued ID
Insurance card
Prior imaging studies (X-rays or MRI) on CD or digital file if available
Radiology reports for any prior imaging studies
Physical therapy notes or discharge summaries
Records from prior physiatry or pain management visits
Documentation of any spinal injections or other treatments

Imaging Before Your Visit

• If you have not previously had X-rays of your spine, our office will usually obtain X-rays prior to your visit so they can be reviewed during your consultation.

X-rays are often helpful even if an MRI has already been performed, as they provide important information about alignment and spinal stability.
